CMS Incident Report Form

We take bullying and harassment very seriously at Central. While many middle school students can tease each other or say something unkind from time to time, we will address targeted, repeated behavior as soon as we learn of it. An anti-bullying source defines bullying as follows: 
 
Bullying is unwanted, aggressive behavior among school aged children that involves a real or perceived power imbalance. The behavior is repeated, or has the potential to be repeated, over time. Bullying includes actions such as making threats, spreading rumors, engaging in hate speech, or attacking someone physically or verbally.
 
If your child is being bullied or observes another student being bullied, please encourage him/her/them to fill out this Incident Report Form. It is confidential and will only be seen by our administrative/counseling team, Mr. Domer, Ms. Mickelsen and Ms. Jobak. 
 
If your student is experiencing a conflict with a friend or having a social challenge that is not bullying but could benefit from conflict management, please reach out to our admin/counseling team. We will do our best to help your child manage conflicts and challenges he/she/they are having while at school.
